If you have checked the booking calendar and found an available date, please complete the form below, print it, sign it, scan it into a pdf, and email it back to This email address is being protected from spambots. You need JavaScript enabled to view it. .  Alternatively, you can sign it electronically by typing "/s/ (your name)" in the signature lines.  Then just save it to your computer, and then email that file to me.  I will treat the electronic form of the signature as an original, and your use of the form in this manner constitutes your consent to same.  It's your choice, whatever you are most comfortable with.  However, no contract will be formed unless it bears my actual handwritten signature.

Please note that what you submit to me is an offer which I am free to accept, reject, or counter-offer.  We have no contract until we have both signed the agreement.
